ICBs
Clinical Commissioning Groups (ICBs) commission most of the hospital and community NHS services in the local areas for which they are responsible. Commissioning involves deciding what services are needed for diverse local populations and ensuring that they are provided.
CHCs
NHS Continuing Healthcare, (NHS CHC), is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. In order to receive NHS CHC funding individuals have been assessed by Clinical Commissioning Groups (ICBs).
PHBs
A personal health budget is an amount of money to support someone’s health and wellbeing needs, which is planned and agreed between the individual or their representative, and the local clinical commissioning group (ICB).

DISCHARGE TO ASSESS
Discharge to Assess is a service for people who are clinically optimised and do not require an acute hospital bed but may still require care services. They are provided with short-term funded support to be discharged to their own home (where appropriate) or another community setting.” – NHS.uk
